Job Description

Job Title: Case Manager

FLSA Status: Non-Exempt

Reports to: Crisis Services Manager

Job Purpose
Provide trauma-informed case management services to residents of YWCA Shelter.

Primary Duties and Responsibilities
Serve as the primary Case Manager for women with children.
Provide all services in accordance with a woman-defined service model, and MOCADSV Service Standards.
Complete strengths/needs assessments and housing goal plans with assigned residents.
Provide information, referral and advocacy to assist participants with accessing mainstream health and human services programs.
Coordinate and facilitate the provision of services provided by other organizations or professionals.
Provide transportation, as needed, to assist residents/clients with accessing services and community resources. Employee must possess a valid driver’s licensed and insured vehicle with insurance complying with minimum agency standards.
Maintain confidentiality of all service recipients in compliance with state and federal law and YWCA policy.
Conduct safety planning with all assigned residents and review as needed.
Provide information regarding Crime Victim’s Compensation and assist with applications as appropriate.
Oversee data quality to maintain compliance with data standards.
Maintain statistics and complete reports as assigned.
Participate in team meetings, professional development activities and trainings as assigned.
Facilitate support/educational groups as assigned.
Provide shift coverage in shelter as assigned.
On-Call duties as assigned.
Attend staff meetings and training as required and/or assigned.

Other related duties as required and/or assigned.

Qualifications
Bachelor degree in human service field or equivalent education/experience with related case management experience preferred.
Knowledge of domestic & sexual violence dynamics preferred.
Knowledge of community resources preferred.
Excellent interpersonal and communication skills.
Ability to maintain confidentiality.
Ability to relate well to diverse populations.
Valid driver’s license and insured vehicle.
Ability to work flexible hours.

Working Conditions
Forty (40) hour workweek with flexibility regarding days/times to meet client needs, shelter coverage and/or deadlines.
Employee works in an office environment and is protected from weather conditions and contaminants.
Some evenings/weekends may be required based upon client needs and business demand.
Some on-call responsibilities required on a rotating basis.

Physical Requirements
Standing or sitting for long periods, bending, stooping and occasional lifting.
Repetitive hand motion (such as typing)
Ability, on a consistent basis, to perform activities requiring cooperation, instruction, persuasion or speaking with others
Ability to communicate effectively in person with consumers

Technological Aptitude: Understand software applications and maintain skills necessary to effectively perform key responsibilities through use of company utilized programs.
